FWC Admission Criteria

In an effort to ensure that our patients receive the best care appropriate to their needs, it is necessary for us to provide criteria for admission. We do not discriminate on the basis of gender, race, social background, religion or creed.

Priority is given to patients who require:

  • Hospice Care:

    Life Limiting condition with a prognosis of six months or less if the disease runs its course.

  • Palliative Care:

    Specialized medical care that focusses on providing patient relief from pain and other symptoms of a serious illness, no matter the diagnosis or stage of the disease.

Disease Specific Criteria

  • Cancer

  • Heart disease

  • Liver disease

  • HIV and Aids (Palliative Care)

  • End stage kidney disease (Excluding Dialysis)

  • Progressive neurodegenerative disorders

    FWC Hospice is not a specialized Psychiatric Home and therefore applications for specialized conditions such as Psychiatric, Bi-Polar, Schizophrenia and related conditions cannot be considered for admission.

    We are unable to admit any patients who are diagnosed with or display symptoms of Alzheimer’s and Dementia.

    • Doctor’s & Social Workers Report  (Current)
    • A pre-assessment will be scheduled at home or at our care centre at a nominal fee.
    • Assessment outcome will determine admission.
    • Accommodation
    • 24/7 Nursing Care
    • 5 Nutritious Meals/Snacks per Day
    • Laundry Service for patients
    • Transport to and from Helen Joseph Hospital/Westbury Community Clinic for chronic and other visits (A referral needs to be done should the patient not belong to the above mentioned hospital/clinic prior to admission. Should the patient need to be treated by a different Hospital/Clinic this will need to be done by the family member
    • Physical therapy, Occupational Therapy, Speech and Audiology , either outsourced to government hospitals or private consultations at additional cost.
    • Supply of adult diapers
    • Chronic Medication Collection (if arranged prior to admission with FWC Manager)
    • Recreational Activities

    Monday to Sunday: Between 11:00 – 15:00

    30 minutes in designated Patient Area

    Only two visitors allowed per patient
